Neuro Plasticity does not refer to plastic or clay inside of your brain. It does refer to the remodeling of the brain however. Definitions are, the capability of the nervous system to adapt or modify to imposed change or the ability of the brain to rewire itself from a portion of the brain that died from having a stroke to a new trainable portion of the brain. Neuro plasticity can be a major player in ones recovery from stroke. These neural pathways will start to relocate immediately after a stroke. One might say, well that's not working for me, I had my stroke years ago". Even years after having ones stroke neuro plasticity can still work. So why does it seem not to work or  work only with difficulty? The main reason is it takes time to regrow those nerves and retrain the muscles and other brain functions. Do not give up trying, even when you try to think about moving a part of your body, you are causing this process to happen faster. People who have undergone hemispherectomies, (removal of one half side of ones brain) have demonstrated function relocation and have retrained the remaining portion of the brain to do what old portions used to do. Because development of these neural pathways will only grow microns a day. this process will usually take a substantial amount of time. Without appropriate therapy optimal functional recovery is not usually achieved. It is like the old adage use it or lose it. I hate to say the brain is like a muscle, but it is true the more activity you give it the better it gets. This can be applicable for memory retention, limbs, vision and speech recovery. You can recover most, if not all, of what you lost because of your stroke. I know of this personally, because I did it..
